Get the operation in november and switch to higher franchise after the end of the year - same effect.
Though I finally could google a result that should clear it up (in German):
In short: You can switch, but franchise can only be adjusted yearly.

Well if you know a year in advance that you have that operation or if you are at 300… Most of the people that want to use this trick are usually on the 2500 franchise and then want to do a quick switch to 300 to go back to 2500.
